Dear Alejandro,
I think that this incessant VT should be hemodynamically stable. A drug that
seems very promising, especially in cases like this, with a great failure of
the pump, probably in part secondary to tachycardiomyopathy, is carvedilol
(Hiroi Y, Fujiu K, Komatsu S, et al. Carvedilol therapy improved left
ventricular function in a patient with arrhythmogenic right ventricular
cardiomyopathy. Jpn Heart J. 2004; 45:169-177.)

In this situation, radiofrequency could and should be attempted. Recently,
Silva et al, from the Section of Arrhythmias of the "Hospital Clínico de
Barcelona" under the direction of Josep Brugada, from a great cohort of 403
patients affected with electric storm, 13 had ICDs and were treated with
radiofrequency (Silva RM, Mont L, Nava S, Rojel U, Matas M, Brugada
J.Radiofrequency catheter ablation for arrhythmic storm in patients with an
implantable cardioverter defibrillator.Pacing Clin Electrophysiol.
2004;27:971-975.) The approach pathway was endocardial and epicardial in 3
patients. In 80% of the cases (12) the procedure was successful. In a
follow-up of 12 months, only 2 patients presented recurrence.

There are references of ICD implantation with 7 months in a child with 6 kg
and 60 cm, who was a little patient carrier of LQT3 refractory to drugs. ICD
was implanted by the team of Prof. Wilde in Amsterdam (Ten Harkel AD,
Witsenburg M, de Jong PL, Jordaens L, Wijman M, Wilde AA. Efficacy of an
implantable cardioverter-defibrillator in a neonate with LQT3 associated
arrhythmias. Europace. 2005;7:77-84).

Prezado Alejandro:  Penso que esta TV incessante deve ser hemodinamicamente
estável. Uma droga que parece muito promissora especialmente em casos como
este com grande falha de bomba em parte provavelmente  secundária a
taquicardiomiopatia é o carvediol (Hiroi Y, Fujiu K, Komatsu S, et al.

Nesta situação pode e deve  ser tentado  a energia de radiofreqüência.
Recentemente Silva et al, da Seção  de Arritmias do Hospitla Clínico de de
Barcelona comandado por Josep Brugada, de um grande cootor de 403 pacientes
afetados de tormenta elétrica 13 e com ICD foram tratados com
radiofrequencia (Silva RM, Mont L, Nava S, Rojel U, Matas M, Brugada
J.Radiofrequency catheter ablation for arrhythmic storm in patients with an
implantable cardioverter defibrillator.Pacing Clin Electrophysiol.
2004;27:971-975. ) A via de abordagem foi endocárdica e epicárdica em 3
pacientes. Em 80% dos casos (12) o procedimento foi bem sucedido. Em um
seguimento de 12 meses apenas 2 pacientes tiveram recorrência.

Há referencia de implante de ICD com 7 meses em criança de 6kg e 60cm em um
pacientinho  portador de LQT3 resistente as drogas. O ICD  foi implantado
pela equipe do Prof Wilde de Amsterdam  (Ten Harkel AD, Witsenburg M, de
Jong PL, Jordaens L, Wijman M, Wilde AA. Efficacy of an implantable
cardioverter-defibrillator in a neonate with LQT3 associated arrhythmias.
Europace. 2005;7:77-84.).
