Dear Deborah,
Firstly, as Co-Director of the Symposium, I appreciate your considerations
about it.
As you will understand for sharing with us, doctors, the daily struggle
against diseases and pain, it gets very hard to give answers to personal
concerns regarding someone's own health and that of beloved ones via e-mail,
and even more so, to do it in a public Forum of scientific discussion.
Nevertheless, I will try to approach your needs by providing you with some
ideas, which must not replace at all, those of the colleagues in charge of
your family's follow up.
First, I would comment that there are some points of connection between ARVD
and Brugada Syndrome, so much so that a prestigious European very
experienced school denies the existence of this last syndrome, placing all
of them under the title of ARVD.
Secondly, let me tell you that the complications that may arise in the
evolution of women carriers of ARVD5 are less worrying than the ones seen in
men.
Also, the prophylactic implant of an ICD in patients without spontaneous
arrhythmias, in whom only TV/FV was induced during the EP study, may
dramatically modify and in a favorable way, the prognosis and evolution of
these cases.
Lastly, in a few words, I would just tell you that both on the Brugada
Syndrome side (your brother-in-law) and on your sister's, (who has ARVD),
the children of both should have the studies performed that were widely
commented in the Forum.

I hope that somehow, these brief comments answer your doubts.

En primer lugar, en mi caracter de Co-director del Simposio, le agradezco
por
sus consideraciones respecto al mismo
Como Usted comprendera por compartir con nosotros los medicos la lucha
diaria
contra la enfermedad y el sufrimiento, se hace muy dificil dar respuestas a
preocupaciones personales que incumben a la propia salud y a la de seres
queridos por e-mail y mas aun, hacerlo en un Foro publico de discusion
cientifica.
No obstante, intentare aproximarme a sus requerimientos acercandole algunas
ideas, que para nada deben reemplazar a aquellas que deba hacer a los
colegas
que estan a cargo del seguimiento medico de su familia.
En primer lugar le comentaria que efectivamente existen puntos de contacto
entre ARVD y el Sindrome de Brugada, al punto que una prestigiosa escuela
euopea de gran experiencia niega la existencia de este ultimo sindrome,
colocando todo bajo la egida de la ARVD.
En segundo lugar dejeme decirle que las complicaciones que pueden surgir en
la
evolucion de mujeres portadoras de ARVD 5 son menos preocupantes que las que
se observan en los hombres.
Asimismo, el implante profilactico de un CDI en pacientes sin arritmias
espontaneas, en quienes solo se indujo TV/FV durante el estudio EF, puede
modificar dramaticamente y en forma favorable el pronostico y la evolucion
de
estos casos.
Por ultimo, en pocas palabras le diria solamente que tanto por el lado del
Sindrome de Brugada (de su cuñado) como por el lado de su hermana (que tiene
ARVD) los hijos de ambos deberian realizarse los estudios que fueron
comentados ampliamente en este Foro.

Espero que de alguna manera estos breves comentarios respondan a sus
inquietudes.
