I completely agree with the comments of Dr. Riera.  His empahsis of
the shortcomings of the nonrandomized retrospective study design is
appropriate.  Nevertheless, the data from such studies are used in
clinical science to develop hypotheses worthy of testing (such as the
COMPACT study described by Dr. Riera) and in clinical practice to
inform decision making while definitive trials are underway.
