我认为药物无效而症状较重的患者应用消融治疗是合 
理的。消除症状的纪律很大。所以可以不必等到RAAFT 
结果出来后在开展这一治疗,因为这类患者现在没有 
好的治疗选择。
这里对于消除症状有另一问题需要解决,正像PCI对冠 
心病仅解决症状一样,导管消融对房颤的治疗作用也 
是如此。这一手术可以减轻患者症状,虽然并发症机 
会可能较高,对于症状难控制的患者消融治疗是合理 
的。

I think ablation for highly symptomatic individuals who are
refractory to
antiarrhythmic drugs is quite reasonable. The odds of eliminating
symptoms are
very good. So there is no need to wait for the completion of RAAFT to
start
doing the procedure in these patients for whom the alternatives are
poor. In my
experience though these patients are rare.
The difficulty comes when one mistakes the elimination of symptoms
with a
cure. Ablation is no more a cure for AF than PCI is a cure for
coronary  artery
disease. The procedure is palliative and has significant
complications.  But
palliative treatments are reasonable for those with intractable
symptoms.
