我有一个对于年轻无器质性心脏病患者,存在持续性 
房颤时电复律前的抗凝问题.

通常对于持续性房颤持续时间大于48小时的患者转率 
前我们应该正规抗凝.但是这一点似乎又与另一观点 
矛盾,就是指南中对于年轻无器质性心脏病患者本身 
由于栓塞风险低,可以不进行抗凝治疗.

对这种电复律前存在的特殊状况有相关应用的证据吗?

Dear colleagues,
My questions is about the need of anticoagulation prior to persistent
atrial fibrillation that will be cardioverted electrically, and is
present in young people without structural heart disease

As a rule we anticoagulate the patient if PAF lasts more than 48 hs,
or else, if we ignore the onset; but this is a contradiction in part
to the stand of not anticoagulating chronically young patients with
PAF without structural heart disease.

There is any special situation in pre-electrical cardioversion
therapy that would justify this use?
