Congratulations for such spectacular symposium. I also would like to thank your unconditional teaching through it. So I dare to ask this question:
I have a female patient, 51 years old, chagasic, with permanent pacemaker. Symptomatic bradycardia 5 years ago. She says that since 2 months ago she has dyspnea, FC 2-3. She deemed it unimportant. Today, we casually found AF with moderate ventricular response during the control for her pacemaker, with echo that does not show significant alteration. She is asymptomatic to this date. She consulted with her electrophysiologist, who suggested anticoagulation with acenocumarol, and later electrical cardioversion…
My question is:
Is there consensus for such management, taking into account her pacemaker, or should we wait, or start a pharmacological treatment? Why?
Which would be the most appropriate management to follow?
Thanking you in advance,
DR. PÉREZ SOLIZ
