Dear Jeff,
Do you place the epicardial ICD percutaneously under floroscopy? or
echo or go transvenous through the the heart veins in the cath lab?  
Because
of the size and anatomical constraints, we presume the percutaneous  
approach so, in brief what's TCH, or your, favored approach?
