15E RE:肥厚型心肌病衰竭期女性患者 －－Kadish医师

她可能有家族性心肌病。这类患者中某些呈进行性心 
脏阻滞和左室功能不全，这种情况下，可能与肥厚型 
心肌病有关；其他呈进行性心脏阻滞的患者中，可能 
发生房间隔缺损（ASD）。

此时，单单植入起搏器有发生心脏猝死的危险，当 
然，对某些严重的左室功能不全和心脏传导阻滞的患 
者，我建议植入ICD＋CRT，可望改善心功能并防止心脏 
猝死的发生危险。

15E RE: A female with burned-out phase of HCM? Dr. Kadish

She appears to have a familial cardiomypopathy. Some cohorts of

these patients have progressive heart block and LV dysfunciton. In

this case HCM may be associated. In other patients with progressive

heart block, an ASD may be present.



In  any case there is a risk of SCD with pacemaker therapy alone.

Certainly, in the presence of severe dysfunction and CHB, I would

suggest an ICD CRT device with the hope of improving function and

protecting aginst the risk of SCD.
