I think I somehow disagree with the experience of CRT in children.
We are not people with a lot of experience on CRT in children, but the
cases in which they were implanted, we had no doubt of the benefits and
their survival. What I do believe is that they are much more controlled 
and cared for, and that is why I think our response was not that bad. I 
do believe that we should, as in all cases, place the left electrode the 
best way possible, and in a good lateral vessel…not just placing the 
electrode carelessly, and this is where I think the difference of 
results in the case may be. Personally, with children, I am much more 
demanding with the lateral or postero-lateral vessel, and I check, 
whenever possible, with early echo the possibility to adjust by Quickpot 
or by echocardiographic control of AV and PV delay, as well as VV delay.
I think these are not the ideal cases, but when they do show up, I do 
not hesitate to place them if I’m going to give them the chance. I am 
clear in that we should choose the best option possible for the case, 
and I equally agree that we should not implant a device when there is 
narrow QRS, since probably the improvement will be pyrrhic.

everyday to do things better.
