Dear Dr Artuza,
 
Anecdotally, my impression has been that patients with secondary 
pulmonary hypertension do not do well after CRT. I was, however, 
surprised to find a clinical improvement (NYHA class III to II) as well 
as a reduction in echo-derived (from the tricuspid regurgitation jet) 
pulmonary artery pressure from over 100 mmHg to < 40 mmHg. I have no 
explanations for this. However, it is possible that the tricuspid 
regurgitation was mediated via right ventricular dyssynchrony and that 
the latter was somehow corrected by CRT. I am due to do a repeat right 
heart catheterisation and I will keep you posted.
