Sorry for the delay in replying.I have been away from my office this past
week.
 You are correct that I did not mention the specific details of enhancing
the visibility of the epsilon waves in the paper I published with Guy
Fontaine in 1995.  The specifics are present in the paper published in
conjunction with Dr. Indik and is part of this symposium. The use of double
speed and double amplitude to record the epsilon waves increases the
amplitude and the 40 HZ filter decreases the noise.   Our hospital has
Burdick Ecg machines and it is not difficult to change the filter settings.
I suggest you contact your technical represenative to get instructions as to
how to do so, if possible.
If not, try to decrease baseline noise using the regular settings after
carefully preparing the skin under the V lead electrodes to obtain a
artifact free baseline.  Frank Marcus

Ud. esta en lo cierto: no mencione los detalles especificos del aumento de
visibilidad de las ondas epsilon en el trabajo que publique con Guy Fontaine
en 1995. Los detalles especificos se encuentran en el trabajo publicado en
conjunto con el Dr. Indik, que es parte de este simposio. El empleo de doble
velocidad y amplitud para registrar las ondas epsilon aumenta la amplitud y
el filtro de 40 Hz disminuye el ruido. Nuestro hospital tiene máquinas ECG
Burdick y no es dificil cambiar la calibracion del filtro.
Le sugiero ponerse en contacto con su representante tecnico para obtener
instrucciones sobre como hacerlo, si es posible.
De no ser asi, intente disminuir el ruido basal empleando la calibracion
normal luego de preparar cuidadosamente la piel bajo los electrodos de las
derivaciones V para obtener una linea basal libre de interferencias.

Greetings and congratulations for the Symposium.
I would like to ask Dr. Marcus about the ECG with modified leads. I thought
that ECG with modified leads was conducted with a common electrocardiograph
at double speed and amplification, but in your reply you mention that it
must be conducted with a 0,5Hz – 40Hz filter. I did not know about this
technical specification, that is why I read Dr. Marcus and Dr. Fontaine's
works again (Marcus, F. PACE 1995; 18:1298) and I did not find any mention
about that technical specification on the text; and on that same article the
bibliographic reference for the ECG with modified leads is a work of
Fontaine et al. (ARVC. A review. Cardiomyopahty update, University of Tokyo
Press, 1994; 4:247) which I could not access.
I would like to get more technical information about how to make that
modification in the common electrocardiograph (is there some trade mark or
model that allows making those changes, or have you ordered for one to be
made for you?). Another question, what percentage of patients with epsilon
wave in the modified leads has positive late potentials in the SAECG
(especially, if we use 25 Hz as "high-pass filer")? In other words, is
epsilon wave in modified leads not a "primitive" variant of late potentials
in the SAECG? Aren't you recording the same?

Quisiera preguntarle al Dr. Marcus sobe el ECG con derivaciones modificadas.
Pensaba que las derivaciones modificadas era realizadas con un
electrocardiógrafo común al dobre de velocidad y amplificación, pero en su
respuesta menciona que las mismas deben realizarse con un filtro de 0,5Hz
-40Hz. No tenía conocimiento de esta especificación técnica , por lo cual
volví a leer el trabajo de los Dres. Marcus y Fontaine (Marcus,F. PACE
1995;18:1298) y no encontré ninguna mención a esa especificación técnica en
el texto; y en ese mismo artículo la referencia bibliográfica para el ECG
con derivaciones modificadas es un trabajo de Fontaine y col. (ARVC. A
review.

pude acceder.
Me gustaría más información técnica sobre como hacer esa modificación en el
electrocardiógrafo común (¿hay alguna marca o modelo que permitan hacer esos
cambios, o ustedes pidieron que les construyeran uno por encargo?) Otra
pregunta: que porcentaje de los pacientes con onda ypsilon en las
derivaciones modificadas tienen potenciales tardíos positivos en el SAECG
(en especial si usamos 25 Hz como "high-pass filter"). Dicho de otro modo,
la onda ypsilon en las derivaciones modificadas no es una variante
"primitiva"
de los potenciales tardíos en el SA-ECG?. No están registrando lo mismo?
