[ARVD-FORUM] Fat in the RV myocardium. Dr. Fontaine

Another important document to illustrate Frank Marcus discussion on Fat in
the RV myocardium.
The attached slide was made from the systematic study of 85 patients who
died of a noncardiac cause in a general hospital in Paris. Grading of fat was
made by direct view of the histologic material at low magnification (Published 
in Acta Cardiologica)**** . This work was made by my mentor and friend in 
pathology Fabrice Fontaliran MD. I have made the statistical analysis 
stressing that : 
1. 3.7% of the population shows the quiescent form of RVD pattern with fat and 
fibrosis without arrhythmias
2. 35% have really normal compact RV myocardium without fat and/or fibrosis
3. 60% remaining cases have some degree of fat without fibrosis

As the p value is 0.04, I was forced to consider that pure fat in the RV is
not a "normal variant" but a new form of RV cardiomyopathy. I proposed the
term "Fat Dissociation Syndrome (FDS)" [Dissociation Adipeuse du Ventricule
Droit (DAVD)] to isolate this particular aspect which carries in its major
forms a risk of Sudden Death probably less severe than typical ARVD but the
risk of RV dysfunction only seen in case of prior LV dysfunction.

When a gave a talk as the invited speaker at the Royal Brompton Hospital in
London some years ago Pr Robert Anderson who came especially to attend my
presentation was impressed. He concluded that this work was very, very, very
provocative and very interesting.
See also the recent work of Mary Sheppard. She was the person who organized
this famous lecture that I should add in in my biography because I was the
only invited Speaker!...

Otro documento importante para ilustrar la discusion de Frank Marcus sobre
la adiposidad del miocardio del VD.
La diapositiva adjunta se realizo con el estudio sistematico de 85 pacientes
que murieron por causas no cardiacas en un hospital general de Paris. La
graduacion de la adiposidad se realizo por observacion directa del material
histologico con baja magnificacion (publicado en Acta Cardiologica)****. Este
trabajo fue realizado por mi amigo y mentor en patologia, el Dr. Fabrice
Fontaliran. Realice el analisis estadistico enfatizando que:
1.      El 3,7% de la poblacion muestra una forma silente de patron de DAVD 
con adiposidad y fibrosis sin arritmias.
2.      El 35% tiene un miocardio del VD compacto realmente normal sin 
adiposidad y/o fibrosis.
3.      El 60% de los casos restantes tiene algun grado de adiposidad sin
fibrosis.

Como el valor p fue 0,04, me vi obligado a considerar que la adiposidad pura
en el VD no es una "variante normal", sino una forma nueva de miocardiopatia
del VD. Propuse el termino "Sindrome de Disociacion de Adiposidad (SDA)"
[Dissociation Adipeuse du Ventricule Droit (DAVD)] para aislar este aspecto
especial que implica en sus formas principales, un riesgo de Muerte Subita
probablemente menos grave que la DAVD tipica, pero el riesgo de disfuncion
del VD solo se observa en caso de disfuncion previa del VI.

Cuando di una charla como conferencista invitado en el Hospital Royal
Brompton en Londres hace algunos anios, el Prof. Robert Anderson que vino
especialmente a ver mi presentacion, quedo impresionado. Concluyo que este
trabajo era muy, muy, muy provocativo y muy interesante.
Tambien ver el trabajo reciente de Mary Sheppard. Ella es la persona que
organizo esta famosa conferencia que deberia agregar en mi biografia porque
fui el unico Conferencista invitado!
La hospitalidad con la participacion del Prof. Richard Sutton fue
maravillosa.
