情况。一些患者低血压或者呼吸困难则不适合大剂 
量。而且从小剂量逐渐增加到适当计量是必要的

伴发房颤后，心输出量可下降高达30%，甚至在一些老 
年人下降50%，进而导致呼吸困难，活动后疲乏无力， 
而倍他阻滞剂可以使得上述症状恶化加重。我建议在 
心肌病中谨慎应用倍他阻滞剂也是因为药物本身的应 
用即需要谨慎。

Dr. Ivanhoe,
The guidelines are made according to the experience of the group. I
generally use beta blockers as a routine in HF; however, the posology
depends on the clinical conditions. Some patients do not stand the
high doses of beta blockers by hypotension or left decompensation,
and it is necessary to slowly increase (months) the dose until the
desired level.
With AF, cardiac output gets to decrease up to a 30%, and in elderly
people it reaches 50%, which causes dyspnea or fatigue with efforts
just by chronic AF, and as some beta blockers worsen inotropism, they
complicate the whole set of clinical symptoms of HF. The reason of my
reasoning for caution in cardiomyopathies is because the use of beta
blockers is a phenomenon that we should always consider with caution.
