Dear Dr. Pereira,
I think that the most convenient thing would be to give him a
combination of ASA-Dipyridamole of slow release (Agrenox), which has
shown to be more effective in secondary prevention of stroke than
other medications.
On the other hand, he would have to be given a proton-pump inhibitor
for his gastritis. We should not forget that oral anticoagulation
outside an INR: 2.0-3 is worse than not taking anything.
